Population Health Manager (PHM) is a secure, web-based, fully managed application providing commissioners and public health professionals with the insight and intelligence required to become World Class. PHM enables Primary Care Trusts to:

  • Aligned to the World Class Commissioning agenda, PHM enables Public Health and Commissioning teams within Primary Care Trusts to realise an evidence-based approach to improving local population health outcomes. PHM enables users to:

    Population Health Manager (PHM) provides access to a range of robust, timely and relevant datasets which can be blended to provide a detailed picture of your population's health needs. These include:

    • Hospital inpatient and outpatient activity data (updated monthly)
    • Population demographic data and projections
    • Health and lifestyle prevalence estimates

    PHM is regularly updated with new datasets and benchmarked models which provide new intelligence on local population health outcomes. The tool also enables users to upload, manage, analyse and share local datasets to blend with those core datasets provided with the tool.
